PALLIATIVE CARE
Palliative care is a comprehensive care program especially designed to help you manage physical symptoms and emotional distress related to illnesses such as cancer, heart disease lung disease, kidney failure, cystic fibrosis among others.
The goal of the program is a maximize your quality of life by improving your physical, emotional and spiritual well-being.
This program is provided by your insurance at no cost to you.
Your supportive care team will be:
- Palliative Care Physician
- Case Manager
- Palliative Care Nurses
- Social Worker
- Chaplain
- Pharmacist
When should you consider Palliative Care?
You may want to consider palliative care if you struggle with one or more of the following symptoms related to your illness.

- Shortness of breath
- Fatigue
- Anxiety
- Loss of Appetite
- Problems with Sleep
- Medical treatment side Effects
How can you initiate palliative care?
Inform your doctor that you want to add palliative care as part of your treatment plan and request a consultation.

We work with your primary health care provider to coordinate a plan of care that will offer effective symptom management as part of your treatment.
SUPPORT
Our team is there to provide expert advice and address your physical and emotional health needs, so you can have the best care experience possible.
RESPONSE
We care about your well-being and will work hard to ensure you receive a quick response. You will receive visits from your team routinely and as needed.
EDUCATION
Discussions about treatment choices, including symptom and pain management, will be provided so that you understand your condition, care options and other needs.
